Home-Based Care Services and Community-Based Resources

Patients who have limited or no ability to leave their home may face a constellation of medical, functional, emotional, financial, and social issues. Most have family or other caregivers who need education and support. These patients and their caregivers are best served by teams and resources that can support complex needs in their homes and communities. In this chapter, we first discuss the spectrum of home-based services available for patients, including home-based medical care, skilled services, aides, and caregivers, as well as how one qualifies and pays for these services. We then discuss community-based resources available for homebound patients and how they are funded.
