The anesthesiologic management of neurosurgical patients needs a multidisciplinary approach across all phases of care to enhance surgical outcomes and minimize complications, aiming for the restoration or preservation of normal neurological functions. Central to this approach is optimizing the patient’s preoperative condition to mitigate potential complications and improve postoperative recovery. A unified and adaptable pathway tailored to clinical requirements must be established, encompassing thorough preoperative testing, patient evaluation, and assessment of surgical and anesthesiologic risks. Anesthesia for neurosurgery requires a deep understanding of central nervous system functions, and adaptability to evolving techniques. Challenges in airway management, possibly influenced by cervical spine disease and intracranial pathology, require customized approaches using advanced intubation devices. Careful selection of induction agents, maintenance methods, and monitoring protocols ensures patient safety, especially with intraoperative neuromonitoring. Collective perioperative management strategies, including pain control, must be clear and endorsed by the entire team to promote patient recovery, reduce complications and mortality rates, and optimize cost-effectiveness and length of hospital stays.
