Human papillomavirus (HPV) infection is a pervasive global health concern, with certain genotypes posing a significant risk for various cancers. This chapter explores the prevalence and clinical implications of HPV in head and neck squamous cell carcinoma (HNSCC), emphasizing its impact on diagnosis, treatment, and prognosis. Notably, HPV-positive oropharyngeal squamous cell carcinoma (OPSCC) exhibits distinct characteristics and treatment responses compared to HPV-negative counterparts. Current diagnostic strategies, including p16 immunohistochemistry (IHC) and molecular techniques, are discussed, along with emerging methods such as RNA in situ hybridization and droplet digital polymerase chain reaction (ddPCR). Additionally, the potential of blood-based molecular diagnostics and next-generation sequencing (NGS) for HPV detection in HNSCC is examined. Despite promising advancements, challenges remain in standardizing diagnostic workflows and incorporating novel techniques into clinical practice. Recommendations for optimizing HPV detection in HNSCC are provided, emphasizing the need for regulatory-approved diagnostic tests and comprehensive guidelines to improve patient care and outcomes.
