Patient blood management (PBM) is a comprehensive strategy aimed at enhancing patient outcomes through the responsible and efficient utilization of blood components. By prioritizing the optimization of a patient’s own blood resources, PBM minimizes the reliance on allogeneic transfusions and mitigates associated risks. This approach encompasses various interventions spanning preoperative assessment, intraoperative blood conservation techniques, and postoperative care. The fundamental principles of PBM involve a meticulous evaluation of each patient’s unique transfusion requirements, considering clinical factors and laboratory parameters. This enables the identification of individuals at higher transfusion risk, facilitating proactive measures to address anemia and optimize presurgical hemoglobin levels. During surgery, strategies like minimally invasive procedures, precise hemostasis, and utilization of cell salvage techniques aid in reducing blood loss and limiting the need for transfusions. Furthermore, PBM emphasizes the significance of managing anemia through alternative therapies such as iron supplementation and erythropoiesis-stimulating agents, stimulating red blood cell production and enhancing hemoglobin levels. Postoperative care centers on monitoring and maintaining optimal hemoglobin levels, ensuring appropriate fluid management, and optimizing tissue oxygenation. This chapter provides an overview of normal hematopoiesis and hemostasis, the core principles of PBM, and interventions involved in successfully implementing a PBM program in clinical care.
