Office-Based Outpatient Focal Therapy Under Local Anesthesia

The introduction of image-guided biopsy has revolutionized prostate cancer treatment, facilitating a shift towards focal therapies (FT) aimed at preserving organ structure while treating visible disease. This chapter explores the evolution of an office-based targeted therapy approach over the past decade, highlighting its advantages, such as the avoidance of general anesthesia, expedited execution, precision, and quick recovery. We also discussed anesthesia protocols tailored to minimize patient discomfort, outlined rigorous patient selection criteria, explained detailed procedural considerations to ensure efficacy, safety, and patient comfort, and reviewed adverse events and long-term outcomes to underscore the office-based FT feasibility and efficacy.
