Parenteral nutrition (PN) therapy is a crucial clinical intervention for numerous patients, necessitating formulations tailored to individual nutrient requirements. Typically, PN includes protein in the form of amino acids, alongside carbohydrates and fat as energy substrates. Micronutrients such as vitamins, trace elements, and electrolytes are essential additives to the PN admixture. PN stands out as one of the most intricate intravenous admixtures, comprising over 40 chemical components. It is classified as a high-alert medication due to stringent safety-focused policies, procedures, and systems required to mitigate patient risks. The diversity of PN is reflected in formulations that can be customized based on specific needs, compounding methods, or total osmolarity. Administration through a central line is recommended owing to its hypertonic nature. However, peripheral PN, featuring a larger volume and lower nutrient content, is a viable option for an appropriate non-central line. Two common systems for PN administration are the 2-in-1 system (containing amino acids and carbohydrates) and the 3-in-1 system (containing amino acids, fat, and carbohydrates). Regardless of PN type or compounding method, adherence to guidelines, standards, validated methods, and recommendations is imperative for the safe compounding and delivery of PN.
