Magnetic resonance imaging (MRI) is a highly sensitive imaging technique employed in specific scenarios for screening and diagnosing breast lesions. On MRI, three dominant patterns, focus, mass, and non-mass enhancement (NME) are described. A significant overlap of MRI findings among benign, atypical, and malignant breast lesions results in a higher false-positive rate, leading to additional imaging examinations and biopsies. It is essential for practicing pathologists to be cognizant of the radiology lexicon of MRI findings for radiologic-pathologic correlation and appropriate interpretation of breast biopsies. In this chapter, we apprise practicing pathologists with current indications for breast MRI and review the imaging patterns of common breast lesions encountered in MRI-guided breast core biopsies.
