The dorsum preservation technique can be used for the unproblematic dorsum. I used to choose it for cases like the one shown in Fig. 21.1. No problems are observed in the profile or frontal view, except a slight curve. In the short term, the results were satisfying. However, in such cases, I experienced two main problems after 6 months: Patients complained of both of these problems and were upset. Focus on the depression and splaying expands gradually over time on the midvault, as shown Fig. 21.2. As I have observed in my practice, a hump can recur in some cases, especially when a bony hump is apparent. I present an example in Fig. 21.3. As you can clearly see, the dorsum appears straight intraoperatively. However, 1 year later, a dorsal hump recurred. I also believe that the risk of cerebrospinal fluid (CSF) rhinorrhea is higher than with the dorsal reduction technique. Ultimately, I stopped using the dorsal preservation technique.
