Patient Safety and Quality Improvement

Multiple factors contribute to poor quality of care and unsafe acts: lack of development of a safety culture, lack of staff accountability, and lack of institutional transparency. It is critical for all pediatricians to develop a policy in their clinic or their institution to prevent medical errors from occurring and to always aim for zero errors.
