The Lymphatic System in Kidney and Related Diseases

Renal lymphatics, often overshadowed by the intricate network of blood vessels in renal physiology, have emerged as integral components in maintaining fluid homeostasis and combating disease pathology. This review aims to shed light on the anatomy, physiology, and pathophysiological significance of renal lymphatics in both healthy functioning and disease states. The methods used to gather information include reviewing various journals discussing the renal vascular system and the adjunct lymphatic vasculature. Renal lymphatics serve as the primary conduit for interstitial fluid drainage, a pivotal function in regulating fluid balance within the renal parenchyma and preventing edema formation. Additionally, they actively participate in immune surveillance and inflammation resolution by facilitating the trafficking of immune cells and inflammatory mediators. Dysfunction of renal lymphatics has been implicated in various renal disorders. In conditions such as renal fibrosis, impaired lymphatic drainage leads to the accumulation of interstitial fluid and solutes, exacerbating fibrotic processes and impairing renal function. Similarly, in acute kidney injury, compromised lymphatic function hampers the removal of inflammatory cytokines and cellular debris, prolonging tissue damage and impairing recovery. Furthermore, emerging evidence suggests a role for renal lymphatics in the regulation of blood pressure. Lymphatic vessels modulate renal interstitial pressure, influencing renal perfusion and sodium handling, thus contributing to blood pressure regulation. Dysfunction of renal lymphatics may therefore contribute to the development of hypertension and other cardiovascular complications, such as congestive heart failure. The lymphatic system’s intricate interplay with renal pathophysiology highlights its potential as a therapeutic target for various kidney diseases. Understanding the multifaceted roles of renal lymphatics offers promising avenues for developing innovative interventions. This review underscores the importance of acknowledging renal lymphatics as indispensable components of renal microcirculation. It advocates for intensified research efforts aimed at unraveling the precise mechanisms governing their functions in kidney physiology and pathology. By shedding light on the often-overlooked realm of renal lymphatics, this review attempts to broaden our comprehension of renal biology and stimulate further investigations into harnessing their therapeutic potential.
