Handovers are defined as the process of transferring the clinical care of a patient from a departing to an oncoming provider. This process not only implies the transfer of responsibility and accountability between two medical teams but involves also the exchange of important healthcare information that are essential for the continuity of care. It has been shown that transitions of care between individual providers or teams have a high potential for errors due to incomplete transfer of critical information or information misinterpretation. Pediatric patients represent a particular vulnerable population at high risk for medical errors due to their small weight and size and their particular physiology. The handover process represents a multidisciplinary task, where the different medical teams involved have each different tasks to accomplish in order to complete a correct handover process. It represents a real communication opportunity to prevent near misses. The improvement of the quality of the handover process has been identified as an important factor to improve patient safety. Different tools have been recently developed to improve handover quality and safety. Successful handover communication is an important skill that all healthcare providers need to learn in order to provide safe and effective patient care.
