At the turn of the century, a significant shift in healthcare was observed. As mortality rates from critical illness improved, it became evident that healthcare itself was contributing to morbidity and even mortality in our patients. In the ICU, hospital-acquired infections, bedsores and drug errors were no longer seen as unavoidable difficulties, but as avoidable harm. This marked a fundamental shift in our understanding of how to improve healthcare.
