T2W hypointensity of the pituitary gland is infrequent and must draw attention. In adults, it is defined by comparison with the normal signal of the gland or with that of the cortex of the temporal lobe. T2W hypointensity is rare in prolactinomas and could correspond to a particular PitNet with a higher prolactin level and sometimes a different response to dopaminergic drugs. in acromegaly, T2W hypointense adenomas correspond to densely granulated tumors which are smaller than the sparsely granulated ones, invade rarely the cavernous sinus, and are good responders to medical treatment. T2W hypointense hyperproteinic nodules are pathognomonic of Rathke cleft cysts. Hemochromatosis and amyloidosis can also demonstrate a T2W hypointense signal of the pituitary gland.
