Immunotherapy and Prostate Cancer: Harnessing the Immune System for Treatment

Prostate cancer (PC) remains a significant health challenge globally, with distinct clinical presentations in various populations. In Africa, PC often presents at an advanced stage, thus limiting treatment efficacy and patient outcomes. Immunotherapy, which employs the body’s immune system to identify and eliminate cancer cells, was developed as a targeted alternative to conventional treatment approaches. The role of immune checkpoint inhibitors, therapeutic vaccines, and adoptive cell therapies in activating the immune response against PC cells is still under investigation. However, the effectiveness of immunotherapy in an African context, the challenges, and the potential discovery of new innovative treatments within this specific demographic need further studying and understanding. This chapter will therefore discuss clinical responses observed in subsets of African patients undergoing immunotherapy. The chapter will delve into the disparities in treatment access, genetic variations, and tumor heterogeneity within the African population which necessitate a more refined approach to optimize immunotherapeutic strategies. The benefits and challenges of the current immunotherapies in prostate cancer care, particularly focusing on the African demographic characterized by a higher prevalence of late-stage diagnoses, will be explored.
