Genomic Discoveries: Precision Medicine and Personalized Approaches in Prostate Cancer Care

Prostate cancer (PC) patient outcomes vary considerably due to the heterogeneous nature of the disease, underpinned by molecularly distinct PC clones. The underlying molecular mechanisms contributing to this clinical heterogeneity have not been fully elucidated. Therefore, a better understanding of this molecular, functional, and clinical complexity is urgently needed to identify the determinants of poorer outcomes. Classical prognostic biomarkers and histopathologic approaches currently available for screening and diagnosis are not sufficient to determine the clinical complexity associated with PC. Despite undergoing definitive therapy to eradicate PC, patients often respond differently to therapies, and the emergence of treatment resistance in high-risk patients is common. All this is assumed to be attributed to the genetic variants between individual patients and their respective tumors. The sub-Saharan African (SSA) region has realized higher mortality in PC; as such, understanding the genomic landscape of PC in an African context has a potential to elucidate the relationship pathological disparities of the disease. Recent advances in high-throughput profiling of genomic alterations in PC offer the possibility of better understanding the molecular basis of PC heterogeneity, revealing molecularly defined PC subtypes that could better inform treatment decisions. In this way, genomic-informed treatments may, in future, enable precision approaches to target specific tumor vulnerabilities and cancer pathways. This chapter discusses the importance of genetic profiling and molecular characterization in guiding treatment decisions and predicting patient outcomes. Furthermore, we explore the exciting advances in targeted therapies that offer improved treatment efficacy and patient outcomes.
