Urinary Continence and Erectile Function in the Salvage Setting

Robotic surgery has expanded in the last 20 years, and salvage robotic-assisted radical prostatectomy (S-RARP) has become more common in centers with access to robotic surgery. However, despite the technological advancements, S-RARP still challenges surgeons and patients due to its suboptimal results. In this scenario, our chapter summarizes the current state and outcomes of S-RARP and describes the opinion of a referral center in this type of surgery. We reviewed several studies conducted in recent years, highlighting trends and factors influencing potency and continence outcomes. Studies reveal that potency and continence recovery following S-RARP are suboptimal and can be influenced by patient-specific factors such as age, preoperative erectile function, urinary function, and surgeon experience. Additionally, surgical techniques, including nerve-sparing procedures and advancements in robotic technology, have positively impacted post-S-RARP outcomes. In this context, we highlighted the importance of personalized treatment plans and preoperative counseling for patients undergoing S-RARP and the need for ongoing research to refine surgical techniques and optimize patient outcomes.
