Widening the Reach of Cognitive Therapy with Trial-Based Cognitive Therapy Techniques

In this chapter, Dr. Irismar Reis de Oliveira, the developer of Trial-Based Cognitive Therapy (TBCT), elucidates this novel therapeutic model intended to enrich the scope of cognitive therapy for transdiagnostic patients. TBCT integrates a unique three-level, three-phase conceptualization diagram, addressing automatic thoughts, underlying assumptions, and core beliefs. The model utilizes a legal metaphor and the Trial technique to challenge and modify these cognitions, providing an integrative approach that combines cognitive restructuring, behavioral experiments, and experiential techniques. This method encourages a more profound therapeutic change across a wide spectrum of mental health disorders. The chapter offers a comprehensive overview of TBCT’s theoretical foundations, methodology, stages, and specific techniques to alter cognitions at each level. Empirical validation of TBCT’s efficacy in managing depression and anxiety symptoms is explored, based on a systematic review and meta-analysis of 10 Randomized Controlled Trials involving 1245 participants. The findings demonstrate that TBCT’s efficacy and dropout rates are comparable to other first-choice psychotherapies. Consequently, TBCT emerges as a promising, flexible alternative in the psychotherapeutic landscape for mental health disorders.
