Medical Treatment of Obesity

Obesity is a significant health problem leading to major complications. The treatment of obesity involves a multifaceted approach aimed at managing a complex interplay of genetic, environmental, behavioural, and psycho-social factors contributing to weight gain. Lifestyle interventions, including reduced caloric intake and exercise, form the cornerstone of management but the clinician must recognise the limitations of this in higher risk patients. More intensive management, including pharmacotherapy or bariatric surgery, can be considered in such high-risk patients under careful medical supervision. The GLP-1 agonists have become the pillar of medical management of the condition and offer multiple other cardiovascular and metabolic benefits. However, their benefits must be weighed against their cost and their side effect profile. In severe cases, bariatric surgery is a viable option, yielding substantial and sustained weight loss in addition to metabolic improvements. Behavioural interventions, including cognitive behavioural therapy, are often incorporated into these management strategies. Despite advancements in therapy, the treatment of the condition remains challenging and should be tailored to the individual, their preferences, their comorbidities, the cause of their obesity, and their complications from the condition. Early referral to a multidisciplinary speciality service comprising of both physicians, bariatric surgeons, dietitians, psychotherapists, and clinical nurse specialists should occur in severe cases.
