Safety Causation Analysis in Sociotechnical Systems

In this introductory chapter on safety and accident causation analysis in sociotechnical systems (STSs), the historical evolution and theoretical foundations of STSs are explored. Originating in the 1950s at the Tavistock Institute, the concept of STSs is challenged by the traditional technological imperative, emphasizing the dynamic interplay between social and technical elements. Individuals are recognized as valuable resources rather than mere extensions of machines within these systems. Key safety-related characteristics of STSs, such as feedback loops and emergent properties, are considered essential for accident prevention and understanding system complexity. Real-world examples of STSs across industries are further examined, underscoring the importance of safety and the far-reaching consequences of accidents. Human factors, including human errors, cognitive biases, and organizational culture, are acknowledged as playing significant roles in accident causation within STSs. Additionally, the challenges of conducting causality analysis in these complex systems are addressed, where data collection and thorough investigations can be hindered by the dynamic nature of STSs and the involvement of multiple stakeholders. This foundational knowledge equips readers to approach system analysis in STSs comprehensively, with a focus on proactive safety measures and resilience-building to ensure a safer future within these intricate systems.
