In microvascular surgery, achieving precise and patent arterial anastomosis is crucial, but can be especially challenging in situations where access to arterial space is hindered by anatomical obstructions or trauma, or when the preservation of vascular branches is necessary. Traditional methods, such as the end-to-end two stay sutures technique, often require flipping the clamp over, which is not always possible or can make the procedure difficult in small spaces. The endto-end one-way-up technique offers an alternative by allowing sutures to be tied first on the posterior wall before proceeding to the anterior wall, thus eliminating the need to flip the clamp. This chapter will provide a detailed, step-by-step description of the end-to-end one-way-up microsurgical anastomosis technique using the rat femoral artery model.
