Surgery plays a pivotal role in the management of adrenocortical carcinoma. Unfortunately, there is no clear, widely adopted or shared definition of the technical steps of a correct oncologic resection. This chapter discusses the different technical aspects of an accurate adrenalectomy for cancer, based on the guidelines and the relevant literature. Similarly, it addresses the difficult topic of the surgical strategy to be adopted for recurrent disease, with special focus on the management of tumor-bed and peritoneal recurrences.
