The initial intra- and extratemporal anatomy of the facial nerve is described and illustrated. The elements of the pathophysiology of facial nerve injury are outlined, and the clinical applications are discussed. Reconstruction of facial paralysis is detailed, including X-face nerve grafts and pedicled and microvascular muscle transfer. Four clinical scenarios are discussed.
