A Community-First Technology Model to Help Ameliorate Social Determinants of Health

Background: Emerging technologies, including interactive health applications, can play a powerful role in advancing health. However, not all patients and communities have access to health technologies, and not all clinicians know how to effectively deploy technologies to enhance the services they provide. Purpose: To offer an integrated technology model, aimed at ameliorating social determinants of health, that nurses may leverage in their work with communities. Method: We discuss the features of the 2TEAM2GO® Model, and we share two use cases designed within the model’s framework—the OH-I-CAN® and Pedia BP® applications. Results: The OH-I-CAN® and Pedia BP® applications have been tested, implemented, and scaled up across various community settings. Both have positively impacted clinical outcomes, patient and provider education, and policy reform efforts. Discussion: The 2TEAM2GO® Model is a flexible and effective framework to guide nurses and nurse technologists in their efforts to ameliorate SDOH in collaboration with communities.
