One-Stage Revision Total Knee Arthroplasty for Periprosthetic Joint Infection

One-stage exchange is indicated when there is no sign of systemic sepsis and in cases where a pathogen has been isolated. In a recent systematic review, a reinfection rate of 12% after an average follow-up of 58 months was found. The most common causative pathogens were gram-positive bacteria (71%), gram-negative bacteria (7%), and polymicrobial infections (8%). The infection-free survival following management for repeating infection was 92%. The causative pathogens at reinfections differed from the primary infection (gram-positive 44%, gram-negative 11%). Individuals who experienced a one-stage revision total knee arthroplasty for periprosthetic joint infection exhibited a reinfection percentage inferior or comparable to other surgical treatments as two-stage or debridement, antibiotics, and implant retention.
