Isolated Versus Full-Component Revision in Total Knee Arthroplasty for Aseptic Loosening

摘要

Isolated tibial polyethylene insert exchange (ITPIE) seems to be a dependable treatment for instability and/or polyethylene wear following total knee arthroplasty (TKA) in the presence of well-fixed and well-aligned components with good survival and clinical improvement. The less invasive nature and diminished risk for bone loss made it an attractive alternative versus full revision. Regarding partial-component versus full-component revision TKA (rTKA), it has been reported that isolated and full-component rTKA for aseptic loosening does not differ with respect to prosthesis failures, adverse events, and clinical outcomes at 5 years. Besides, poor American Society of Anesthesiologists (ASA) status, increased comorbidities, instability, and a severe bone defect have been related to worse functional improvement.
