Evolution and Revolution in Pituitary Medicine and Surgery

The evolution and revolution of pituitary medicine and surgery are explored in this chapter, shedding light on the transformative changes that have shaped the field. Beginning with a historical overview, the chapter traces the origins of pituitary surgery to ancient Egyptian techniques and early endoscopic advancements. Key pioneers such as Sir Victor Horsley, Davide Giordano, Francesco Durante, and Alfred Hirschmann are highlighted for their significant contributions. Various surgical approaches, including Schloffer’s transsphenoidal approach, Kocher’s submucosal dissection approach, and Hirsch’s endonasal approach, are discussed in detail. The impact of technological and imaging advancements, such as the introduction of the lighted nasal speculum, intraoperative fluoroscopy, surgical microscopes, and the rise of endoscopic techniques, is examined. Additionally, the chapter explores the role of medical advances, including hormone discoveries, molecular biology, and pathological diagnosis, in advancing our understanding and treatment of pituitary disorders. The advantages of endoscopic techniques over traditional approaches are highlighted, while ongoing advancements and future directions in the field, such as the nasoseptal flap technique, combination therapies, and emerging technologies, are also discussed.
