Carotid-Ophthalmic Artery Aneurysms

Carotid-ophthalmic artery aneurysms are rare but potentially life-threatening vascular lesions. They are typically located at the junction of the internal carotid artery and the ophthalmic artery and can cause visual impairment, neurological deficits, or subarachnoid hemorrhage. Despite advances in endovascular techniques, microsurgical clipping remains the mainstay of treatment for carotid-ophthalmic artery aneurysms due to its high obliteration and low recurrence rates. However, surgical clipping can be technically challenging due to the complex anatomy of the carotid-ophthalmic segment and the proximity of critical structures such as the optic nerve, chiasm, and pituitary gland. Preoperative planning, microsurgical techniques, and intraoperative monitoring are essential for the success of the surgery. The use of temporary occlusion, intraoperative angiography, and intraoperative monitoring can help to reduce the risk of complications and improve the outcome of the surgery. Patient selection is also critical for the success of the surgery, and careful consideration should be given to the age, comorbidities, and neurological status of the patient. Despite the challenges associated with the treatment of COAs, microsurgical clipping remains a viable and effective option for the management of this rare but potentially devastating vascular lesion.
