Children presenting with drug-resistant focal epilepsy respond poorly to medical management. In contrast, epilepsy surgery offers significantly improved seizure outcomes and quality of life. Within the procedures available to the pediatric epilepsy surgeon, hemispheric resection/disconnection surgery in selected patients is a safe and effective technique. This surgical technique comprises several steps, and here I discuss the nuances of the insula disconnection with a view to optimizing efficacy and safety.
