Although there is a paucity of studies examining program startup directly, it is common experience that a well-structured ECMO program is likely to have benefits that include cost savings, improved safety, program resilience, and improved outcomes. There are several ways to start a program depending on size, available financial and other resources, and market demands. Each method has pros and cons that must be evaluated for each institution, but for most institutions, starting small and growing incrementally is recommended. Components of a program include a well-defined leadership team, structured and documented processes and procedures, equipment management, quality control, and team members with well-defined roles. Of these roles, the bedside specialist and their various staffing models are of particular importance to understand. Long-term viability and health of the program depend on defining measures for success, gathering and responding to internal data, and understanding and responding to the wants and needs of the institution.
