Immunotherapies for Cancer: Bi-specific T Cell Engagers (BiTEs)

Bi-specific T cell engagers (BiTEs) are immunologically active molecules with dual specificity, concurrently binding to a tumor-associated antigen and CD3 on T cells. This approach brings cytotoxic T cells and tumor cells into close proximity, thereby intensifying immunological responses against neoplastic cells. Originally employed in the context of minimal residual disease in B-cell precursor acute lymphoblastic leukemia, various novel BiTEs have been approved or are currently in development for the management of multiple hematologic and solid malignancies. BiTEs may also present logistical advantages over chimeric antigen receptor T cell (CAR-T) therapies, as they circumvent the intricate procedures associated with autologous T cell genetic engineering and eliminate the necessity for supplementary cytotoxic chemotherapy while harnessing endogenous immune potential. As evidenced by an expanding spectrum of indications and demonstrated efficacy, BiTEs emerge as a promising anticancer therapy, capitalizing on the intricacies of the immune system while evading the logistical challenges associated with CAR-T therapies.
