Charcot arthropathy of the shoulder is a chronic progressive destructive process of the joint and periarticular tissues. It is caused by loss of pain sensation in the joint due to neurological disorders such as tabes dorsalis, leprosy, diabetes mellitus, and syringomyelia. The typical presentation involves unilateral painful or painless shoulder, with accompanying swelling and possible restriction of range of motion, and a history of trivial trauma. Neurological manifestation symptoms may be subtle and include a loss of sensation to pain and temperature (hot or cold) in the upper extremity and upper thorax. Early diagnosis is essential to prevent treatment failures.
