The management of patellar tendinopathy is challenging, especially in season. Athletes often wait to seek treatment for several months or years after symptom onset, when the symptoms and impairments are debilitating. Several treatment options have been described for patellar tendinopathy, with progressive tendon loading exercises being the intervention with the highest level of evidence. In this chapter, we will describe key aspects on patellar tendinopathy, from pathophysiology to assessment, and rehabilitation until return to sport.
