Current Concepts in Ligament Augmentation: Suture Tape Augmentation in ACL Repair and Reconstruction

Reconstruction of the anterior cruciate ligament (ACL) poses challenges in the intraoperative and postoperative periods. Concerns surrounding graft tensioning, postoperative graft rerupture, loss of lower extremity strength, and/or decreased range of motion (ROM) persist despite decades of research into the optimal ACL reconstruction and rehabilitation technique. Ligament augmentation techniques (LATs) have been present for over 100 years, but it is only in the past several decades these commercially available implants have become frequently used. Historically, total ligament prostheses (TLPs) were used in lieu of traditional auto- or allograft ACL reconstructions. Suture tape augmentation (STA) is a more recent advancement, and its use has increased dramatically over the past 10 years. Biomechanical data suggests STA offers load-sharing capabilities that increase load to failure, but outcomes data directly comparing patients with versus without STA across all graft types is lacking long-term follow-up. Bioactive scaffold suture tapes are now entering the market and bring with them a whole other host of considerations. This chapter provides a summary of the literature surrounding STA, its advantages, its disadvantages, and the authors’ preferred STA technique in ACL reconstruction.
