Rotator cuff repair continues to result in high rates of retears and poor tendon healing despite important surgical techniques. Heightened attention to understanding basic biologics of tissue healing has led to interest in tissue regeneration. Augmentation agents include biologics such as platelet-rich plasma (PRP), stem cells, nonstructural patches (e.g., Bovine Achilles collagen graft), and structural patches (e.g., allografts and xenografts). A large number of studies have been published; however, more studies are needed to determine the true efficacy and clinical usefulness of such agents. The purpose of this chapter is to review biologic augmentation in the form of nonstructural and structural patches, indications, outcomes, and complications associated with arthroscopic patch augmentation for rotator cuff repair.
