Tendon and muscle tissues have limited healing capacity, and incidence of unsatisfactory functional outcomes of repair are driving the search for alternative therapeutic approaches. Promotion of healing and regeneration process is the main scientific rationale to use orthobiologics therapies. Understanding of the complex cellular environments during tissue healing could promote development of new successful therapies. However, optimization strategies are at an early stage, and great importance has been given to identify key molecular involved. Tissue engineering is considered but none can yet be considered conclusive in their reproduction. We report current available scientific evidence regarding biological interventions, such as stem cells, with special emphasis in their use for tendon healing and regeneration. In vitro and in vivo studies suggest that mesenchymal stem cells (MSCs) exhibit the ability to reconstitute injured tendon tissue and promote tissue healing, with an immunomodulatory effect which may reduce the inflammatory environment. Regenerative medicine technologies, including cellular therapies, gene therapies, and multimolecular preparations of growth factors and cytokines, such as PRP, are expected to advance the field of orthopedics and sports medicine.
