Maximizing Donor Egg Efficiency: Artificial Intelligence and Genetically Certified Oocytes

Oocyte vitrification has become popular especially due to late marriages or delaying having children because of sociological reasons, inability to have children due to genetic and medical reasons, oncological problems, or donor oocyte banking practices. There are many criteria to be considered for the successful application of oocyte vitrification. Especially, vitrification process based on the morphological selection to be made in donor oocyte banking can be successfully applied with the optimization of an appropriate vitrification protocol. The use of AI (Artificial Intelligence) in the morphological selection of oocytes has revealed that morphological classification based on mathematical and morpho kinetic measurements can be successful in practice. In the morphological selection based on the AI system, both the morphological evaluation of the oocyte and the clinical data of the patient or donor can be assessed. Especially, in donor oocyte banking, the inclusion of the donor in the oocyte bank in conformity with the efficiency values obtained from the donor’s previous cycles and the selection of oocytes accordingly will significantly increase the rate of usable blastocysts to be obtained from the donor. As AI-based software will directly affect efficiency values, it will provide an important advantage for patients using oocyte donation also in financial terms. Another thing is that there is the possibility of evaluating the aneuploidy status of oocytes with the PGT-A method before vitrification. In this evaluation, the polar body taken by biopsy method will be evaluated by NGS (Next Generation Sequencing) method, and it will be determined whether it is euploid or not. This will eliminate the risk of aneuploid oocytes, especially for patients who can receive a small number of oocytes for oocyte donation, thus it will provide more euploid blastocysts and a higher chance of implantation and clinical pregnancy. Polar body biopsy is a procedure that requires experience, optimized protocol, and appropriate micromanipulation methods. It has been specified that the procedure is completely safe and survival rate of over 95% will be achieved after thawing with an appropriate vitrification protocol. Vitrification, thawing, and ICSI protocols of these oocytes should be optimized, and a non-standard way should be followed.
