Year 1: Internal Medicine Learning Using “1,2,3 Methodology”

This section is the start of the most comprehensive sections of this book and forms the scaffolding of this learning strategy. It tests grit and resiliency while building the foundation for a dedicated learner. The target is to learn eight to ten “pearls of knowledge” each day from conferences and attending rounds. The most demanding time of training is during inpatient medical wards consisting of 1 day on-call, a day post-call, and 1–2 days of off-duty wellness. Organization is the key to allow for three dedicated days each week for intensive review and self-education of materials. As such, this section lays out a detailed 3-year, 3-days-per-week plan to tackle the often-overwhelming amount of information trainees must learn. The “1,2,3” method is reflected in these 3 days a week. The pearls are of varying lengths given some material in internal medicine can be addressed quickly, while other material needs development in view of a broader context. The varying patterns of material depth aid learners in preparing for the internal medicine examinations, which feature concise questions and assessment of extensive stems, laboratory methods, and radiological features of conditions. Throughout this 3-year study plan, pearls are reiterated and presented in different lights, essential for making connections and absorbing information. The first year section focuses heavily on cardiology, gastroenterology, pulmonary, and critical care medicine.
