Differential Diagnosis and Assessment of Comorbid Disorders in Children and Adolescents

This chapter explores disruptive mood dysregulation disorder (DMDD) in children, characterized by pervasive irritability and difficulty in managing frustration. It discusses the challenges in accurately diagnosing DMDD due to its symptom overlap with other pediatric psychiatric disorders. Sagar-Ouriaghli et al. (Eur Child Adoles Psychiatry 27:1095–1104, 2018) question the utility of DMDD as a distinct diagnosis, highlighting the risk of clinicians overlooking treatable disorders. The chapter also examines the potential benefits of DMDD diagnosis in providing targeted resources and optimizing outcomes. Althoff et al. (Psychol Assess 22:609–617, 2010a; J Am Acad Child Adoles Psychiatry 49:1105–1116, 2010b) and Liebenluft (Bipolar Disord 22(2):195–196, 2020) contribute to the understanding of DMDD’s long-term implications, noting its association with a higher risk of psychiatric disorders and functional impairment in adulthood, but not necessarily leading to manic states or bipolar disorder. This highlights the complexity of DMDD and the importance of differential diagnosis and personalized treatment approaches.
