Frailty in Nursing Homes

摘要

Whilst older people aim to live at home for as along as possible, a period of life in a nursing home might be necessary for many, especially as frailty becomes more severe. At times, it might be for respite or restorative care, at other times, it could be for permanent care. Frailty is prevalent amongst nursing home residents, placing them at-risk for poor health outcomes such as hospitalisation. Just like in the community, residents can improve health wise and possibly avoid hospitalisation where there is proactive as oppose to reactive care. Interventions that might benefit residents following comprehensive assessment include multi-component exercise, medication optimisation, socialisation, healthy diet and advance care planning. Education and research continues to be important for this population group. Improving the quality of care delivered in nursing homes should remain a priority for all societies. Such focus will help residents live their life with quality and dignity.
