The PFFS is a simple and time-efficient tool that uses visual images to assess a person’s level of fitness-frailty. It captures the multidimensionality of frailty by evaluating 14 domains for a total score of 0–43. The PFFS can be completed by the individual or a care partner. Additional versions of the PFFS are formatted for assessment by a healthcare professional and for assessment of both the usual and acute health states. The PFFS has demonstrated high validity and reliability in a variety of settings and patient populations, including in geriatric ambulatory care, for those with cognitive impairments, surgical patients, and older adults with ethnically and racially diverse backgrounds. Overall, the PFFS is a valuable tool that is accessible across literacy levels, languages, and cultures and may improve care planning. It allows crucial elements of an individual’s health—elements that become important to assess when people live with any degree of frailty—to be readily incorporated into the health record.
