Geriatric Models of Care: Perioperative Optimization of Senior Health (POSH)

The Perioperative Optimization of Senior Health (POSH) model is an innovative collaborative care model, which operationalizes best practices in perioperative care of older adults. The model has been shown to improve postoperative outcomes for high-risk older adults and decrease healthcare costs for this population. It can be adapted to the local conditions and resources available in different medical centers. The implementation of the model requires the engagement of interdisciplinary stakeholders and availability of geriatric expertise. Integration with programs such as the Geriatric Surgery Verification Program could further disseminate best practices in geriatric perioperative care throughout larger health systems.
