Mr. Smith is one of the over 22 million older adults with complex chronic conditions who need daily support. Similarly, Mrs. Smith joins the approximately 50 million individuals who provide extraordinary care to one or more family members due to significant disease states or disablement [1]. The number of older adults in the United States (U.S.) living with complex conditions requiring daily support and long-term care will increase to over 38 million by 2030, with the prevalence of those with cognitive impairments more than doubling from over 6 to more than 13 million over this time as well [2].
