Patient Priorities Care: Priorities Aligned Decision Making for Persons with Multiple Chronic Conditions

For older adults with multiple chronic conditions (MCCs), a disease-based, siloed approach may result in fragmented care with contradictory recommendations and burdensome combinations of medications, diagnostic tests, self-management tasks, and specialist visits [1]. The benefits and harms of guideline-recommended treatments are often uncertain because of the effects of coexisting conditions and treatments. Furthermore, older adults, particularly those with MCCs, are excluded from many evidence-generating clinical trials. When faced with trade-offs inherent with MCCs, patients vary in their desired health outcome goals and in what they are willing to do to achieve them [2]. These challenges can make decision-making difficult for clinicians.
