Fall Prevention Intervention Model in Primary Care: STRIDE

Falls are the leading cause of injury-related deaths [1] in older Americans [2]. Many older adults fall each year and experience serious secondary health problems. Approximately one in four older adults fall annually, and 20% to 30% fallers sustain moderate-to-severe injuries [2, 3]. In the USA, these fall-related injuries result in approximately 30,000 deaths, three million emergency department visits, and 800,000 hospitalizations each year.
