Acute Care for Elders

In this overview of the Acute Care for Elders (ACE) Unit model of care, we describe the prevalence of hospital-associated disability (HAD) and the ACE Unit model as an intervention designed to prevent HAD and related hazards of hospitalization. We further define the key elements and operational features of ACE Units and published outcomes in varied patient populations. Finally, we share steps and lessons learned in developing, sustaining, and disseminating ACE models of care.
