A Case of Scapholunate Instability Treated by Arthroscopic Dorsal Capsuloligamentous Repair (ADCLR)

A case of scapholunate (SL) instability in a professional tennis player 6 months after the trauma is presented. The diagnosis was suspected by the preoperative clinical assessment (pain, loss of range of motion and grip strength, positive provocative tests) and the imaging (bilateral comparative dynamic X-rays and arthro CT-scan) . After SL instability was confirmed and staged intraoperatively by the arthroscopy, a dorsal capsuloligamentous repair technique was performed (ADCLR). The mid and long-term clinical outcome were satisfactory, and the clinical assessment showed a stable wrist. Number of open or arthroscopic procedures are described for SL instability treatment and there is no current consensus on the optimal technique according to the instability grade. The ADCLR was chosen in this case instead of other available techniques because it is a mini-invasive and reproducible technique. The series already published shown satisfactory results. Moreover, it does not cut the bridges to secondary reconstruction techniques if needed.
