Chronic, Reducible Scapholunate Ligament Injury: Bone-Ligament-Bone (BLB) Reconstruction

Surgery is often indicated for patients with scapholunate (SL) ligament injury to prevent secondary joint collapse and subsequent degenerative wrist arthrosis. While there are many surgical options, active patients with chronic, dynamic scapholunate ligament injury are candidates for bone-retinaculum-bone reconstruction. The donor site is the extensor retinaculum/periosteum which is locally available from a dorsal approach to the wrist, consistent between patients with limited stretch characteristics, and creates a strong construct. This technique is favorable for its lack of donor morbidity, ease of autograft harvest, exclusive bone-to-bone healing, and surgical simplicity.
