The transhiatal esophagectomy was developed in response to the higher complication rates associated with the transthoracic esophagectomy. Limitations to the transhiatal approach include higher anastomotic leak rate, lower lymph nodes resected, and the need for blind dissection of the mediastinal esophagus. Our minimally invasive transhiatal approach on the Da Vinci Xi platform emphasizes the key technical steps of the open approach, while addressing some of the main limitations. We are able to perform the mediastinal dissection under direct vision, resect higher numbers of lymph nodes, and have lower wound complications.
