Penile Doppler ultrasound (PDU) is a comprehensive tool in the evaluation and management of erectile dysfunction (ED), with an important clinical application for enhancing diagnostic accuracy, informing treatment decisions, and improving patient outcomes. The technique utilizes ultrasound technology to measure blood flow dynamics within the penile arteries, providing valuable insights into the vascular health of the penis. By assessing penile hemodynamic parameters, PDU aids in the accurate diagnosis and classification of this condition. This chapter explores the most relevant indications for performing PDU. Moreover, it discusses the interpretation of PDU findings, which can guide clinicians in tailoring personalized treatment strategies for patients with ED. As research in this field continues to evolve, PDU remains a valuable tool in the arsenal of clinicians, offering a noninvasive and reliable means of investigating the vascular basis of ED and guiding targeted therapeutic interventions.
