Limitations of Penile Hemodynamic Studies with Doppler Ultrasound

Penile Doppler ultrasound (PDU) is undoubtedly an important diagnostic tool in assessing penile vascular health and erectile dysfunction (ED). However, as any other diagnostic test, PDU has its own inherent limitations in capturing the comprehensive nuances of erectile function, which will be discussed in this chapter. By exploring the intricacies of these limitations, this chapter emphasizes the importance of employing a contextual approach to interpret findings accurately and formulate targeted treatment plans. Some of these limitations include clinical-radiological dissociation, severe cavernosal atherosclerosis, and cases of geometric ED. It is important to integrate clinical parameters to bridge the gaps created by PDU’s limitations whenever possible. While PDU has its constraints, it remains an essential tool that, when coupled with a comprehensive assessment, enriches the diagnosis and management of ED.
